PROTEGER: artificial intelligence in the service of geriatric oncology

In Latin America, more than 90% of older adults with cancer do not receive adequate treatment because of the shortage of specialists in geriatric assessment. This exposes patients to serious risks such as toxicity, disability or even death. That is why we at Oncoger set out to build PROTEGER: a solution that uses artificial intelligence to transform care in geriatric oncology.

What is PROTEGER?

PROTEGER is a platform built on artificial intelligence that assists care teams in deciding on cancer treatment for older adults. The tool weighs multiple factors — such as functional status and health conditions — to recommend the safest and most effective treatment.

With a personalized approach, PROTEGER offers three kinds of recommendation:

• Standard treatment.
• Adjusted treatment.
• Avoid treatment because of high risk.

How PROTEGER works

Advanced predictive analysis: PROTEGER uses models such as Random Forest, XGBoost and linear regression, combined in an ensemble that reaches over 90% accuracy in dose recommendations.

Local, validated data: Trained on more than 300 local cases and validated on more than 100 new ones, the model holds its relevance and accuracy in the Latin American context.

Clustering for patient segmentation: The platform uses techniques such as K-Nearest Neighbors to identify patterns and group patients by risk level and by their particular characteristics.

An intuitive interface: Designed to fit easily into clinical routine, PROTEGER presents the recommendations and highlights the key variables behind each one.

Why PROTEGER is different

High accuracy: The eligibility model reaches an AUC of 0.84. The figure was published and peer-reviewed in the Journal of Geriatric Oncology (2026).

Local fit: Designed specifically around the data and the needs of the Chilean and Latin American health systems.

Scalability: Currently in technical validation at more than 7 hospitals in Chile, with the potential to expand to Mexico, Colombia and Peru.
